Pricing: Prices subject to change at any time.
Order Changes or Cancellations: Since stock orders are processed immediately for same-day shipment if received before noon (12:00 pm), changes or cancellations cannot be made. Custom orders may be changed or cancelled within 24 hours if not already processed. For returns, please see Product Return Procedures below.
Payment Terms: Net 30. Terms are available to MADA members only. Non-members may prepay for their order or have it shipped COD (check or money order only). All COD orders are subject to an additional $13.50 fee per order by UPS, included in the amount due.
Credit Policy: MADA members may charge to an active account. Past due invoices for any MADA department may delay current shipping requests.
Product Return Procedures: Stock items may be returned within 30 days with prior approval. Contact MADA Forms Department for approval and return method. We grant credit for returns only if the product and the packaging are in resellable condition. Items returned are subject to a 25% restocking fee.
Warranty: Neither MADA nor MADA Services Corporation makes any express or implied warranties as to these forms.
Shipping Errors: Any shipping error should be reported immediately to MADA Forms Department.
Product Damaged in Transit: If you receive a product that has obviously been damaged in transit, please refuse the delivery. If you notice the damage after the carrier has left the premises, please contact the carrier and MADA Forms Department immediately.
Repackaging Product for Return:
- All products must be returned in their original packaging when possible, including Styrofoam or other other packing materials.
- We cannot give credit on any stock item when either the item or the manufacturer’s boxes have been written on or otherwise marked in any way or if shrink-wrapped packages have been opened.
- All product must be shipped in protective outer boxing.
Freight: Shipping charges are based on weight, destination and type of delivery service. Our goal is to ship by the quickest, most cost-effective means available. Stock items are shipped in 1 to 3 business days when in stock. Delivery time for custom forms is normally 2-3 weeks. Specialty items may take longer. Handling charges of $5.00 per package will be added to shipping charges for packing materials including orders picked up or delivered.
